Vaginal Infections & Imbalances

The vagina is a warm, moist environment and infection-causing bacteria tend to love just such places. The vagina has excellent built-in protectors against infection, primarily in the form of lots of “good” bacteria that create a highly acidic environment. Occasionally, these protectors are overcome. You may notice a change in discharge odor or appearance, pain or itching, a new bump or irritation on your vulva, or something just “feels off down there.” Your symptoms may be due to a sexually transmitted infection (see STI screening and treatment), a recent lifestyle or medication change, ineffective hygiene, or no discernible cause. 

 

Two of the most common vaginal infections and imbalances are:

 

  • Yeast infection – Not really an infection, but an imbalance. Yeast is a normal fungus in the vagina, but too much growth causes problems. An overgrowth can occur from too much sugar in the diet, hormonal changes, heat and moisture, antibiotics, stress, and other lifestyle causes. 

 

  • Bacterial vaginosis (BV) – Occurs when there is not enough “good” bacteria in the vagina, which leads to an overgrowth of problem-causing bacteria. Some people seem to be more prone to BV than others. BV seems to be related to changes in the vaginal pH due to lifestyle (diet, medication, clothes) and/or partner semen. Recent evidence suggests that BV can be passed between sexual partners though males do not show symptoms. It’s important that both partners be treated so that the female reduces her chances of having a repeat imbalance. 

 

At your visit, the midwife will take a health history, complete a physical exam of the affected area, and if possible obtain a sample with a swab for testing. 

 

Both yeast and BV are effectively treated with prescription medication. The midwife can also provide medication for your partner in specific cases. The midwife might suggest use of a pre- or pro-biotic to support the growth of good vaginal bacteria.
